The CEC is the only Center of Excellence in Endometriosis Surgery in North America offering, to our knowledge, free records review and case evaluation.  We're pleased to offer this service to our friends in the endometriosis community.

NEW REGISTRANTS PLEASE NOTE:

Every registration form submitted (below) is responded to via email with an information packet and our necessary requirements for free records review. Please allow 2-3 business days for response.  Please take care to input your email address correctly, double-checking that you have given us accurate information.  We cannot reach you if you provide us with an email address that does not work or does not accept email from us.  If you do not receive your requested information within 2-3 business days, it has been bounced by your server.  Please contact us toll free at 866-733-5540 to provide alternative delivery information as soon as possible.

ALL PATIENTS PLEASE NOTE:
If you are sending videos along with your case file for us to consider, be advised that the tape MUST be properly labeled with your full name, date of birth and phone number.  Videos which do not bear this information will NOT be considered during the case review.  Thank you.

The CEC will return medical records upon request at no charge.  Should you require the return of your case file, please contact us at 770-913-0001 or include a note indicating your request in your records review file.  Please be further advised:  we purge files one year out from receipt of records.  The records of those individuals who do not become CEC patients within one year of sending their case for our free review and evaluation will be destroyed, unless they specifically request return of those materials within the one year time frame.  Thank you.
